County plans diabetes program

Sparta — Sussex County will offer a new self-management program beginning on Tuesday, May 11, for those dealing with or at risk for diabetes,

The Sussex County Department of Human Services, Division of Senior Services is sponsoring the program which will be held at the Sparta Health and Wellness Center.

The program will run from 10 a.m. to 12:30 p.m., on Tuesdays, May 11, 18 and 25, and June 1. 8 and 15.

Based on the evidence-based Chronic Disease Self-Management Program developed at Stanford University, this series of six goal-oriented workshops is facilitated by trained volunteer leaders.

Participants will learn how to identify and manage symptoms; develop healthy eating and exercise habits; ensure proper foot care; communicate effectively with medical professionals; manage fear, anger, and frustration; relax and think positively; and prevent or delay complications.

The program is offered under Title IIID of the Older Americans Act, as amended, at no charge. The workshops are complemented by a free manual, “Living a Healthy Life with Chronic Conditions.”
